Interagency Coordination Process:

The process through which children and their families access services involving the collection of relevant information with regard to the child and family’s needs. The Interagency coordination process links families with appropriate services and resources in a confidential manner.

Service Coordination:

A service coordinator may be the initial contact person with a family once a referral has been made. The coordinator promotes communication between the family and community providers, agencies, and other resources. In addition, the service coordinator provides information regarding services, advocacy, system processes, confidentiality, and family rights.

Interagency Early Intervention Screening Committee:

The team may consist of parents or guardian, the service coordinator, a representative from the school district, a representative from the county health agency, and others as needed. The team reviews information about the child and initiates a single plan process for serving the child/family most appropriately.

Individual Family Service Plan (IFSP):

An IFSP is a written plan that describes in detail who, what, where, when, and how the types of services will be provided to a child and his/her family who qualify for special education early intervention. The plan identifies the frequency of services, when it will be reviewed, funding sources for services, and service coordination for children birth to age three. Parents are asked to provide input into the service plan and must approve of the plan.

Individual Education Program (IEP):

And Individual Education Program or “IEP” is a written plan for providing services to a child over the age of 3 years who has qualified for special education. The plan is specific for the child and details who, what, where, and how services are to be provided. Parents are asked to provide input into the service plan and must approve of the plan.

Interagency Individual Intervention Plan (IIIP):

An IIIP is the same as an IEP. The difference is that it is a written plan for providing services to a child who has qualified for special education when two or more agencies are involved with working with the same child. The purpose of an IIIP is to promote the coordination of agencies’ programs and services and a single plan is developed rather than multiple plans by multiple agencies.
